I/We have read and understand the information provided in this form and give permission for my child to participate in Christina Iaboni’s cooking/baking class.

I/We consent to permit Christina Iaboni to gather my/our child’s personal information for the Cooking/Baking class, including allergies, emergency contact and medical history for use in the case of a medical emergency.

Acknowledgment of Risk

I/We, hereby warrant that my/our child is physically and psychologically fit to participate in cooking/baking activities and understand that the choice to participate brings with it the assumption of  risks, including but not limited to:

• Use of sharp utensils and kitchen equipment (knives, graters, peelers, etc.)
• Exposure to heat from ovens, stoves, and other appliances
• Risk of burns, cuts, slips, allergic reactions, or other injuries
• Possible exposure to food allergens and cross-contamination

I understand that all reasonable precautions will be taken by instructors to ensure a safe environment, but I accept full responsibility for my child’s participation in this activity.

Medical Authorization
In the event of an injury or medical situation, I authorize the program staff to administer first aid to my child. In the event of an emergency, I further authorize the program staff to obtain medical treatment for my child, including transportation by ambulance if deemed necessary. I agree to be responsible for any costs associated with such first aid or medical care.

Behaviour

Inappropriate behaviour, such as the use of abusive language or aggressive behaviour (hitting, punching, etc.) will not be tolerated. Children must also stay in the designated workshop area. Parents will be called to pick up their child is inappropriate behaviour persists.

Photography/Videography

I/We understand that photographs and/or videos may be taken of my/our child and may be used for display, publications and promotion for Christina Iaboni, RD. If you do not want your child’s photo used, please inform Christina Iaboni.

Class Cancellation Policy

Christina Iaboni, RD reserves the right to cancel classes due to low enrollment, sickness, or any other reason. In the event of a cancellation, a refund by e-transfer will be provided.

Refund Policy

We understand that plans can change and that children may occasionally become unwell.

If you are unable to attend a scheduled class, please notify Christina as soon as possible. Refunds will be granted.

Refunds, when applicable, will be issued via e-transfer. Any service or processing fees incurred at the time of registration will be deducted from the total refund amount.

Please note that no refunds will be provided for no-shows.

By registering for the Kids Cooking Class and providing your email address, you consent to being added to our email marketing list. You may receive updates, promotions, and information about upcoming classes.

You can unsubscribe from these communications at any time by following the unsubscribe link included in our emails.

Waiver and Release

In consideration for my child being allowed to participate, I hereby release, waive, discharge, and hold harmless Christina Iaboni, its owners, instructors, volunteers, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may occur while participating in the cooking class.

I understand that this release applies to any injury or illness resulting from participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.
